Bringing your completed paperwork to your first appointment lets us spend the full hour with you, not at the front desk. If you’d prefer to fill these out in person, that’s fine too — please arrive 15 minutes early.
What to bring to your first visit:
- Completed registration forms (download below)
- A photo ID
- Your insurance card
- Your physician’s referral or prescription (if you have one)
- A list of current medications
